a) The Urban Physio requires a minimum of 24 hours notice to cancel or change an appointment. Clients are liable to pay the full amount if they fail to attend, cancel or change their appointment with less than 24 hours notice. The cancellation policy applies even if a courtesy reminder email for any reason has not been received from The Urban Physio.
b) For ACC covered Pilates appointments, clients are required to have a pre-existing ACC claim number AND/OR be assessed for coverage under ACC. Private rates will apply for those appointments where it is deemed that a session does not meet the criteria to be covered by ACC.
c) Clients are liable to pay all payments immediately following their appointment or notification by The Urban Physio that payment is required (e.g. declined claims). All outstanding payments must be paid prior to attending a subsequent appointment.
d) Payments which are included within the Agreement to Pay and Cancellation Policy include:
- Any service fee for a private treatment or co-payment charges for ACC covered treatments.
- Additional costs for any treatment that does not meet the coverage criteria or is declined by ACC or other funder.
- The costs of materials such as orthotics, products, or other goods etc. supplied with the services the client receives.
e) Should a debt recovery service be required to be engaged to recover any debt, the Client will be liable for any recovery fees (including all legal or court fees) on an indemnity basis.
